To join FICAP, select one of the following application formats and return the application, with full payment, to the FICAP office.

1. Fillable Excel Document  - This is an MS Excel file, which allows you to save your typed information.
    

2. Printable PDF Document - This is a PDF file. If you do not have Acrobat reader, you can down load it from the Adobe Web Site.

 
Please be sure to include your check, made payable to FICAP, in the amount of $1500 for your first years' annual dues or complete the credit card information at the bottom of the form. In addition, all FICAP members are encouraged to join the Political Committee (PC).
